Main duties of the job

This is an exciting opportunity for an experienced Registered Mental Health Nurse looking to join the Anglesey Community Mental Health Team. The role involves working with Primary Care Mental Health Services in providing assessments and interventions as defined by Part 1 of the Mental Health Measure 2010. You will need excellent communication skills, be self-motivated, proactive and committed to the promotion of innovation for service delivery.

The successful candidate will hold a caseload of individuals aged 18+ with Mild to Moderate mental health issues. They will work collaboratively with Primary Care partners (General Practitioner, Advanced Nurse Prescribers) and Third Sector Agencies in providing care and appropriate signposting in order to meet the needs of the residents of Anglesey. Intervention will be provided in a time limited manner, and will be recovery focussed. You will also link in with secondary care CMHT, in ensuring that clients with more complex needs are in receipt of the appropriate service.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
