Job summary

The Specialist Support Nurse is responsible for the triage and assessment of acute stroke patients and identify potential candidates for hyper-acute stroke treatments. You will be responsible for the provision of high quality in a 24/7 rota.

You will have an active role in monitoring and improving the standards of care through supporting junior staff, clinical audit, developing and leading practice, contributing to the research and teaching of professional colleagues.

You will be responsible for providing specialist clinical advice within the emergency stroke service and support to patients, families, relatives about their conditions and preventive measures.

You will collaborate with Multidisciplinary teams across stroke service to facilitate pathways of stroke care.

Main duties of the job

  • Assessment, planning, implementing and evaluating care as a practitioner with specialist skills.
  • Deliver TIA and Stroke care based on principles that are research and evidence based and which enhance the quality of patient care.
  • Provide care in a variety of settings including ED, inpatient and outpatient areas.
  • Provide specialist clinical advice for patients and carers regarding diagnosis and stroke prevention.
  • Develop the skills of generalist nurses to enable them to provide a high standard of care for patients with high stroke risk.
  • Provide a high level of communication by:
  • Communicating very sensitive condition related information to patients and relatives, requiring high levels of empathy and reassurance
  • Ensuring the provision of appropriate verbal and written patient information thus empowering the patient to make informed decisions
  • Contributing to consultant led meetings regarding patients within own caseload
  • To work constantly within the framework of the NMC Code of Professional Conduct (2015).
