This is a great opportunity towork across the Humber and North Yorkshire footprint to improve the outcomesfor local people by increasing the detection and management of cardiovasculardisease risks in primary care and other settings.

The post-holders will workwith Place and ICB cardiovascular disease prevention Leads and Local AuthorityPublic Health colleagues to identify successes and challenges, developinnovative approaches to identify those at risk of cardiovascular disease and supportconversations around NHS Health Checks.

The post-holders willwork in the two ICB sub-geographies (North Yorkshire and York, and The Humber)on CVD Prevention and Detection, but where it is sensible to do so, may take alead across the HNY geography on particular areas.

Main duties of the job

The post holders will:

Develop relationships within the ICB, acrossthe Health and Care Partnership, and with relevant regional and national VCSEorganisations to improve cardiovascular disease outcomes.

Work with CVD prevention clinical andprofessional leads across the ICB to develop a strategic approach toidentifying and managing CVD risk

Deliver education and training on CVDprevention, and act as a knowledge source for those working on this agenda

Analyse and interpret complex data andinformation to support decision making where there might be differing views

Monitor outcome measures across the ICB andtake steps to support improvements at Place where these are not achieving the expectedtrajectory

Use the Core20Plus approach to ensure that CVDprevention work explicitly tackles inequalities

Prepare reports and presentations to variedaudiences to demonstrate both successes and challenges to inform and influencerelevant stakeholders

About us

  • Educated to Masters level or equivalent level of experience of working at a senior level in specialist area
  • Extensive knowledge of specialist area, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to masters level equivalent
  • Evidence of post qualifying and continuing professional development
  • Must have an understanding of the background to and aims of current healthcare
  • Should have an understanding of the breadth of primary care, how it is organised, and the potential contribution to CVD prevention
  • Organisations and local authorities.
  • Knowledge of CVD and Long-Term Conditions

Desirable

  • Knowledge and experience of programme management
  • Project management qualification such as Prince 2 or Managing Successful Programmes (MSP)
