The Inclusion Team support and coordinate services for children with additional learning needs, their families and settings/schools to ensure that children access their entitlement with smooth transitions into a range of settings. We work in partnership with learners and their parents, education services, and other professionals in education and health.

About the job

An exciting opportunity has arisen for an Inclusion Transition Worker, working as part of the wider Inclusion Team within the Education Service and with partners in many different agencies. The role focuses on working with parents/carers ensuring that the children with additional learning needs and their families are supported to transition to different settings, access their entitlement and make good progress.
