Most people at some point intheir life may benefit from navigation through encounters with differenthealth services, agencies and professionals, across an often-confusing seascapeof health, social and community care. Effective navigation is a key element of deliveringcoordinated, personalised care and support.
Junior Social Prescribing LinkWorkers can play a crucial role in helping people to get the right support, atthe right time to help manage a wide range of needs. This may include supportwith long-term conditions, social and welfare needs which may be supportedin-house or signposting to a range of statutory and voluntary sector services.
Duties & Responsibilities
Administration duties:
Supportpatients to access appropriate local services; this requires awareness andknowledge of services/providers, how to refer and direct people toward them.This may include working with patients and carers directly with face-to-facetime, with a case management approach either in a GP Practice or Home Visit
Supportwith the delivery of projects, programmes and service improvement when required
Workwith patients and families in a person-centered way, as well as with otherstaff in different services and sectors to help facilitate and link theperson/carer to the right service
Makebasic need assessments and/or follow up and make sure multidisciplinary team(MDT) care plans are fulfilled. At times, more detailed needs and/or riskassessments may be required with developments/adjustments to care plans
Workwithin and closely liaise with an MDT andsupport clinicians to provide timely best care and support
Accurateup-to-date record keeping, creating and maintaining a database of localservices is important
Ensuringpersonalised care plan action points happen, and if not will be able to liaisewith relevant team members
Supportingand enabling healthier behaviours and self-care
Workacross a range of service providers developing and maintain professionalrelationships and network across organisations
Applyproblem solving skills to help create solutions with the person/carer and theMDT personalised ways to support people
Havea sound understanding of theory and principles of person-centred care, longterm conditions and health and social policy
Communication:
The post-holder shouldrecognise the importance of excellent and effective communication skills, withsensitively to different cultures, beliefs and communication needs areessential, making sure every contact counts. The post-holder should strive to:

Reportpatients who behave in an inappropriate manner to the practice manager orproject/service manager
