This is an exciting opportunity for a current band 2/3 to join the colorectal nursing team for a 9 month secondment to cover maternity leave. You will be responsible for teaching and supporting stoma patients in hospital and in stoma clinics and also be responsible for ordering stoma supplies and liaising with community support services.

Main duties of the job
Teach stoma care to patients who have had stoma surgery and where necessary teach there carers how to look after a stoma
Liaise with community stoma care nurses and other care agencies in order to offer continued support for patients with stomas
stock control
Participate in audit data and documentation relating to patients care
Taking phone calls and act upon them

Detailed job description and main responsibilities
- Be responsible for own actions, ensuring a high standard of care is provided and that all patient interactions/observations are communicated and documented appropriately. Provide patient care in a supportive and sensitive manner and refer to other members of the team in order to provide optima
- Demonstrate excellent communication skills with colleagues, patients, relatives, and the multidisciplinary team
- Lead and Implement changes/projects within the department and across outpatients with enthusiasm, motivating, engaging and encouraging the team. Acting in a People supporter Role and/or be involved in the outpatients Shared decision making council
- Demonstrate a focus on promoting patient engagement to provide a more positive experience for our patients and service users
- Demonstrate technical and/or practical skills to ensure optimum patient management g. Stoma care, , IT skills, wound care, where agreed
- Organise own daily workload, ensuring communication is maintained with clinicians and patients, and the colorectal nursing team. Ensuring that stoma supplies are ordered and that areas are wel
- Assist in providing appropriate information to patients for example, information relating to their stoma and its management, providing discharge information and carrying out appropriate referrals upon discharge from hospital
- Attend and contribute to daily patient meetings to support the team
- Organise delivery of stoma supplies for patients upon discharge and organize follow up appointments.
- Perform stoma care as prescribed by clinicians and within scope of knowledge, liaising and communicating with trained staff and attending study/teaching where appropriate. Completing appropriate competencies.
