Job summary
We are looking for an experience knowledgeable band 7 CNS to work alongside our haematology team. The main purpose of the role is to act as an advanced practitioner, providing expert care, treatment and support for patients with leukaemia, myeloproliferative neoplasms, myelodysplasia, anemia’s and their families.
The post holder will be expected to contribute to improving the knowledge of staff within the Trust through education, research and audit as well as developing the practice of others with regard to this patient group. They will be expected to play a key role within the multi professional team in order to ensure a seamless, coordinated approach to care and timely onward referral to other professionals and agencies as appropriate.
You will have recent or current experience of working in an acute setting in the role of a clinical nurse specialist in haematology with a nursing qualification at degree level or above
Main duties of the job
- To underpin practice with the development of robust clinical policies, procedures and guidelines to ensure they reflect the monitoring of patients condition to ensure they are safe to receive procedures such as blood transfusions or chemotherapy
- Serves as a resource to patients and families regarding community resources and support services in order to coordinate care and optimize patients outcomes
- In conjunction with the Matron and the MDT work with the staff in the day unit and inpatient wards, be able to identify and implement strategies for patient and public involvement in service delivery within haematology.
- Participate in Nurse led clinics, this will include prescribing blood products (following training as required) being able to prioritise your workload across the working week
- Communicate with doctors to discuss test results and determine appropriate next steps in treatment plans, this will all you to support patients to make informed decisions during diagnoses and treatment
- Perform holistic needs assessments on patients at key stages of their pathways and document care plans.
- Undertake end of treatment reviews and provide written summaries for the patient and GP.
- Maintain up-to-date knowledge and clinical expertise of all lymphoma and chronic leukaemia treatments.
- Together with other CNS manage the group of patients requiring blood product support.
