As a Care Home Liaison Nurse, you will work in collaboration with colleagues across the system to create conditions for improving the health and well-being outcomes for people who live in care homes.

An exciting opportunity has arisen for a Registered Nurse to work in partnership with the Care Home Professional Support team providing professional support, demonstrated through role modelling, high quality evidence-based care, provision of education, training and development for the care home workforce and monitoring the impact.

We are looking for highly motivated, dynamic, forward thinking registered nurse, with an interest in positively enhancing and supporting care home staff, through virtual and face to face learning experiences by participating in education and development activities as part of our Inter-Professional Care Home Professional Support Team. You will be passionate in contributing to the quality of care provided for older people in care homes through evidence based practice, sign posting, education and improvement, whilst enhancing conditions for a partnership approach which fosters mutual respect, liaison, collaboration and compassion.

You will have excellent communication skills and will work with autonomy and professional discretion to prioritise and determine the daily co-ordination of activities and be proactive in responding and dealing with requests and escalating concerns. 

Full driving license and car owner is essential.
