About us

Hospiscare has been supporting people in Exeter, Mid and East Devon with terminal illnesses for 40 years. Our services include support at home, a specialist palliative care ward, out-patients clinics, and supportive therapies.

We support close to 2,500 local people each year, in some of the hardest times possible. As a local charity, we raise around £8 million each year to continue to provide our care. Our staff, volunteers and supporters help us to care for people when they’re most in need, no matter what their individual circumstances may be.

3. KEY RESULT AREAS/PRINCIPALDUTIES AND Responsibilities

Contribute to the assessment and review ofpatients, prioritising needs and working as a member of the Community team toensure effective communication is in place to support patient care.

Develop advanced communication skills and professionalismin your interactions with patients and those close to them, carers and otherprofessionals in order to promote and maintain Hospiscare values.

Attend and contribute to multi-disciplinary,complex care and Gold Standards Framework (GSF) or End of Life meetings topromote best practice.

Develop knowledge and skills to be able tocontribute in the delivery of education and learning for all those involved inpalliative/end of life care, in order to act as a resource to widerprofessional and care teams both internally and externally.

Contribute to Hospiscares clinical governance andaudit process in order to support continuing quality improvement and learning.

Develop advanced skills in identifying andassessing complex bereavement and communicate effectively with the Supportive Careteam in order to enable a seamless handover into Hospiscares Bereavement CareServices.

Co-ordinate junior colleagues and takeresponsibility for the team in the absence of the CCTL or senior practitioner toensure continuity of service and welfare of patients.
