Our client provides accommodation and support services for people who are homeless or at risk of homelessness across Rochdale, Oldham and Bury. Service users often have complex needs including negative mental health, experience of domestic abuse, substance, or alcohol abuse, learning disabilities or have committed criminal offences. Our client offers personalised support by encouraging residents to make their own decisions and address any barriers with the end goal of residents living independently. Support is available through their supported housing services as well as a central hub which has an open door policy.

We are looking to recruit a Personalised Housing Plan Intervention and Prevention Worker on a temporary basis at their central hub in Rochdale. This role is to start as soon as possible and the working hours are Monday – Friday 8:30am 4pm. The hub provides a free breakfast, showing facilities, on site healthcare, laundry facilities, food parcels, access to clean clothing, one to one assessments, advice and advocacy and links to accommodation services. Outreach Workers also offer support to identified rough sleepers and invite them into the hub to access services they may need.

The role – PHP Intervention and Prevention Worker

As a PHP Intervention and Prevention Worker you will;

  • Provide and coordinate daily support and structured activities that contribute to the holistic support as documented in an individual’s PHP and in line with the Homelessness Reduction Act 2018.
  • Contribute to the completion of and ongoing documentation of an individual’s PHP following assessment from the homelessness assessment team.
  • Identify support needs and empower individuals with complex needs to access appropriate services (including DAAT, Mental health and counselling).
  • Identify and reduce any barriers to accessing relevant support.
  • Provide advice and support to individuals faced with or experiencing homelessness specific to their needs.
  • Plan and facilitate a range of activities and brief interventions that contribute towards the prevention of homelessness.
  • Manage service user journey travelled with effective monitoring and data/outcomes collection in line with data return schedules.
  • Support rough sleepers and those new to homelessness to engage in appropriate support.
  • Work with people with multiple and complex needs to improve their personal health and wellbeing.
  • Enable those with complex needs to undertake and improve their basic skills including, confidence, IT, Reading and Writing.
  • Effectively engage with local partners and support providers to offer appropriate support

The candidate – PHP Intervention and Prevention Worker

  • Experience of working with individuals with multiple and complex needs.
  • An understanding of the barriers faced by those affected by homelessness and experiencing multiple and complex needs.
  • An understanding of the Homelessness Reduction Act 2018 in its approach to preventing homelessness.
  • Experience of delivering support that supports and empowers change.
  • Excellent communication skills including report writing, use of IT and professional representation of the organisation.
  • Ability to consult, identify and facilitate key activities to promote wellbeing.
  • Ability to demonstrate effective partner relationships and multi -agency working.
  • Ability to assess need and deliver effective support appropriate to need.
  • Skill and experience in signposting service users to appropriate services.

Due to the nature of this role an Enhanced DBS is required dated within the last 12 months or on the update service.
