Job summary

This is anexciting opportunity to work as part of a multi-disciplinary team in the SouthBristol Locality with a focus in delivering urgent care. You will be requiredto respond to urgent referrals in need of complex management in order toprevent hospital admissions and facilitate early discharges. You will have continuingresponsibility for proactively monitoring and managing patients on the caseloadto ensure the best outcomes.

There willbe an opportunity to develop your skills in physical assessment and clinicalreasoning with the support of Advanced Clinical Practitioners and wider membersof the team. There is also the opportunity to support the REACT service inpreventing admissions within the Emergency Departments at the two main AcuteTrusts in Bristol (UHBW and NBT).

You will be working out of the Knowle Clinic and TheWithywood Centre.

As a locality we have opportunities for Community StaffNurses specialising in planned nursing care, supporting acutely and chronicallyunwell patients to remain safely in their own homes.

Weekly working pattern:

Up to per week to include some evenings andweekends.

Main duties of the job

As a Senior Nurse in the South Bristol INT time, you will work within a multi-disciplinary team to provide holistic and specialist skills supported by a robust competency framework

Provide specialist, high quality and expert care for patients who require an urgent care response

Work as part of a multidisciplinary team (MDT) to ensure appropriate assessment and care planning.

Support patients to self-manage, monitor their condition and make informed choices

Support service improvement initiatives
