As a Trust we have recently set up a Key Working Team for Autistic Children and Young people (CYP), and/or those with a learning disability, and their families and carers. We seek to provide support for those who are at risk of hospitalisation.

Our service aims:

Navigation – The keyworkers help CYPs, and families, navigate through the complex processes and systems in the community to get the support that they need.

Coordination – The keyworkers work in multi-agency settings to encourage service involvement and make a clear plan of actions for all involved.

Focused on community support – Keyworkers will always work on the assumption that support can be provided at home or in the community. Hospital admission will be a last resort and, should it be essential, discharge will be a focus from the beginning.

Person-centred – Keyworkers deliver flexible, person-centred support. Keyworker plans are coproduced and CYP focussed.

CYP & Family voice – Keyworkers focus on advocating for and encouraging CYP and family voice in all meetings and support work completed.

We are looking to recruit a Highly Specialist Keyworker Lead who will be instrumental in moulding the service to the needs of the CYP, family and carers. If you think you have the knowledge and passion to make a difference to the team then please consider note that JD is subject to job evaluation

Main duties of the job

The post holder would need:

– To love working as part of a team, but also be comfortable leading on initiatives and managing your own workload.

– To be keen to make sure that young people are at the heart of any process and ensure their voice is heard.

– To be a reflective professional with a thirst for learning new things. You will bring relevant experience of working with CYP who have Autism and or a learning disability and are often seen to have behaviour that challenges.

– To work intensively with CYP, their families, carers, and network of professionals.

– To be passionate about working co-productively and have experience/passion for personalised care planning.

– To be able to take a lead role with the keyworkers and to provide them with expertise to assist with their caseload management, and to provide their clinical supervision.

– To be part of the escalation process should further support be required in the team.

– To work closely with the Team Coordinator in order to review and adapt the service in order to meet need.

– To have experience and knowledge that will help the team to improve support for Children and Young People, and their families/carers.

– To professionally adapt their role in order to help with the leadership and development of the team.

  • Professional qualification in relevant field and accredited member of BACP or equivalent level of accreditation with HCPC, NMC, UKCP, CPC, IPTUK, NCS
  • Evidence of specialised continued professional qualification or equivalent experience in relevant clinical practice Autism and/or Learning Disability)
  • Evidence of specialised continued professional training (degree level) in relevant clinical practice (Autism and/or Learning Disability)

Desirable

  • Post graduate qualification in leadership (or considerable experience in this area)
