Our Out of Hours Service is a crucial part of our AdultsSpecialist Services delivering care to people who need it urgently or thosewith longer-term conditions. And, were recruiting a Band 5 Community Nurse tohelp us deliver the best experience possible.

We work on a buddy system which means you will never beworking alone youll always have someone with you for every visit during yourshift.

In this role, youll focus on collaborative, safe, effective and evidenced-based practice. Working closely with primary and community services as well as with social care partners, you play a critical role in ensuring a person-centred experience.

Main duties of the job

In this role, youll focus on collaborative, safe, effective and evidenced-based practice. Working closely with primary and community services as well as with social care partners, you play a critical role in ensuring a person-centred experience.

You will:

  • Assess, plan, implement and evaluate individual, patient-centred interventions drawing on a wide range of specialist knowledge gained from practical experience and specific training.
  • Be competent in a broad range of clinical skills including catheterisation, pain management, wound care, end of life care and IV medication after specific training and more.
  • Actively use and promote shared decision-making, empowering people to improve self-management of their long-term condition while working collaboratively with our teams and system partners
  • Supervise colleagues including; peers, students and unregistered workforce in nursing competencies, helping us shape our existing service and contribute to the future design and delivery.
