HCL Mental Health are looking to cover a placement for a Band 6 CPN to start as soon as possible for 12 weeks, working Mon-Fri 8am – 8pm or 10am – 10pm in an older adults setting in Chelmsford Essex.

Responsible to: Clinical Lead / Matron

Accountable to: Team Lead

Hours of work: 37.5 hours per week (flexible shifts)

Pay: £25 Day / £34 Night (Start from 8pm)

Role Summary:

  • Assessment skills in an acute setting. Preferable some Home Treatment/Crisis/Acute pathway experience. Driver required as home visits to be undertaken across mid Essex.
  • Assess care needs of Older Adults and Adults of Working Age with mental health problems referred to the Mid Home First team.
  • Develop, implement, and evaluate programs of care for these individuals.
  • Work closely with a multidisciplinary team, including Primary Health Care professionals, to provide an integrated and high-quality service.
  • Review service users admitted to the Mental Health Assessment Unit in Colchester and collaborate with In-Patient Services as part of the In-Reach role.
  • Maintain standards of care within the Mid Home First service.
  • Ensure an integrated service is provided to clients and maintain a good inter-disciplinary working environment.
