Job summary

The post holder has a pivotal role within the busy primary care team, which will include supporting the Senior Strategic Head of Primary Care and Transformation and the team, to ensure that their work stream of programmes is planned and managed effectively and take the lead in assisting in their successful delivery.

As a member of the Bradford District and Craven HCP, Place based Partnership of West Yorkshire Integrated Care Board strategy team, the post holder will work as part of a dynamic group including service users, patients, clinicians, and colleagues (internal and external) to deliver the objectives and outcomes of our transformation programmes and projects. Ensuring that the differing needs of the populations and the different objectives and priorities, approach and culture are taken proper account of and reflected in the design, implementation and outcomes of transformation and service improvement programmes/projects whilst addressing areas of greatest need and seeking to reduce health inequalities.

Monitor agreed KPIs and relevant data to measure performance working with provider to highlight any variation and improvement plans Interpret and analyse complex information from a variety of sources, adopting an evidence-based approach to identify areas that require review Page 5 of 14 To liaise and work with a range of stakeholders to obtain complex and sensitive information, collating and reporting the received data to present to senior leaders for future service planning and decision making. This will involve the mapping and reporting of current service delivery requirements, resource provision and utilisation, income streams, operational performance data and processes.
