Chestnut Tree House is the children’s hospice for East and West Sussex and South East Hampshire, providing specialist palliative care for children and young people with life-shortening conditions.

This is an exciting role for an experienced nurse to provide nursing care in the community setting. The post holder will predominantly work in the community, managing a caseload, providing skilled palliative nursing care to all Children and Young People accessing Chestnut Tree House services.

Main duties of the job

There will be opportunities to work across childrenservices and support in-house care. The post holder will have responsibilityfor key working, supporting both children and families living with a lifelimiting condition. This role will suit a dynamic individual who will be ableto deliver respite care in the home, take CYP on fun outings and access daycare at the hospice.

Thepost holder will have a key responsibility in networking and developingcollaborative relationships across health, social care and other voluntaryservices.

The post holder will be supported with acomprehensive clinical skills training programme and will also be developed andsupported to provide end of life care within a 24 hour on call rota in the homesetting.

  • RNC or RN (adult) or RN (learning disability)
  • Continued professional development, including evidence of reflective practice

Desirable

  • Child physical assessment module level 6/7 RN (LD)
  • A recognised teaching and assessing qualification, 998 or Learning Through Mentorship

Experience

Essential

  • Previous Clinical Supervision experience is essential.

Desirable

  • To have had some clinical experience working within a community setting.
  • Experience of teaching
  • Experience of planning, managing and coordinating
  • schedules of care in collaboration with other allied health, social, education and volunteer agencies
  • Evidence of a range of clinical competencies
  • appropriate to the role
  • Experience of clinical audit
  • Experience of palliative care (over 1 years)
