Practice

Assist and perform routine nursing tasks related to patient care as directed by senior nursing staff or GPs and following Policies & protocols within a surgery or in the patients home

o Venepuncture

o BP

o ECG

o Lifestyle Advice

Extended Duties:

Dressings following guidance from Senior Nurses

Spirometry

Frailty

To promote and refer to Social Prescribing

Following agreed clinical protocols with referral to senior nurses/GPs will support frail, elderly, and social isolated to deliver care and support plans promoting health and wellbeing

Document consultations in the patients records via system one

Working with the extended multi-disciplinary team to establish Personalised Care Planning What matters to me for patients with LTCs to help avoid Unplanned admissions

Working with the multi-disciplinary team to support patients living with cancer and promote cancer screening; and provide support to patients living with diabetes, respiratory disease and cardiovascular disease.

Manage workload effectively

Establish links with all practices, the PCN team and connecting with voluntary and services outside general Practice

Participate in the administrative and professional responsibilities of the practice and PCN Team

Ensure accurate notes on all consultations and treatments recorded in their System 1 notes

Recognise and work within own competence

Attend and participate in Practice and PCN meetings

Training & Development

Maintain continued education by attendance at courses as deemed essential for professional development

Participation in annual appraisal review

Maintaining a record of own personal and/or professional development

Governance

Contribute to the assessment of service needs

Assist patients to identify their health needs

Manage and organise individual patient consultations

Aware of identification and reporting procedures relating to professional standards

Health and Safety

• The post-holder will manage their own and others health & safety and infection control as defined in the SEL GP Group Health & Safety Policy, the SEL GP Group Health & Safety Manual, and the SEL GP Group Infection Control Policy and published procedures.
• Comply with SEL GP Group health and safety policies by following agreed safe working procedures
• Awareness and compliance with national standards of infection control, hygiene, regulatory / contractual / professional requirements, and good practice guidelines.
• Correct personal use of Personal Protective Equipment (PPE) and ensuring correct use of PPE by others, advising on appropriate circumstances for use by clinicians, staff and patients.
• Reporting incidents using the organisations Incident Reporting System

The HCA Link Worker must be a car owner with a current clean UK licence. Insurance must cover business use

The PCN operates a no smoking policy

The HCA Link Worker will be expected to wear a uniform provided whilst in clinics or home visits

An identity badge must be worn at all times

Experience

Essential

• -Basic Nursing Skills: Phlebotomy, Blood Pressure, ECG

• – Ability to work autonomously
• – Excellent organisational and time management skills with
• the ability to cope with a busy working environment, with periods of interruption throughout the day
• – Effective written/verbal and non-verbal communication/carers and all members of the MDT and social partnerships

Desirable

• – Extended Skills: Spirometry, Basic Wound Care
• – Knowledge of IT systems to include System 1
• – Understanding the principles of Social Prescribing
• – Knowledge of the Personalised care approach

Qualifications

Essential

• – Educated to GCSE or equivalent (GCSE Maths & English C or above)
• – Demonstrable commitment to professional and personal development

Desirable

• – Training in motivational coaching, behavioural change and goal setting
• – NVQ 3 or equivalent and/or relevant basic/first level professional qualification, or working towards this
• – Health and Fitness qualification L3, Coaching Qualification(s) / Mentoring L3,Health and Social Care L3 Diploma or working towards this
