THE OPPORTUNITY

This role will be hosted by South and East Leeds GPGroup, and be based in the Beeston Primary Care Network (PCN) and Middleton& Hunslet Primary Care Network (PCN)

Thisis an exciting opportunity to provide patients with health and social needs aholistic service working closely with the practices and community services. Wellsuited to an experienced HCA who would like a new challenge to develop extendedclinic skills and learn social prescribing to improve health and wellbeing ofthe individual.

We are looking to recruit 1 full-timeequivalent health link workers.

Main duties of the job

A HCA Link Worker is a role created to complement your clinicalskills with Social Prescribing. You will assist in empowering individuals totake control of their own health and wellbeing as well as performing clinicaltasks. The HCA Link Worker will therefore take a holistic approach connectingpatients to community groups and statutory services for practical and emotionalsupport if required.

The HCA link Worker will workwithin the PCN workforce to support the PCN in the delivery of core services, providebasic Nursing skills and provide social prescribing support for frail andvulnerable patients with long term conditions (LTSs), in accordance with theNHS Long Term Plan.

You will be responsible for workingas part of our PCN multi-disciplinary team to support the

PCN practices under direct orindirect supervision.

Extended Duties:

Dressings following guidance from Senior Nurses

Spirometry

Frailty

To promote and refer to Social Prescribing

Following agreed clinical protocols with referral to senior nurses/GPs will support frail, elderly, and social isolated to deliver care and support plans promoting health and wellbeing

Document consultations in the patients records via system one

Working with the extended multi-disciplinary team to establish Personalised Care Planning What matters to me for patients with LTCs to help avoid Unplanned admissions

Working with the multi-disciplinary team to support patients living with cancer and promote cancer screening; and provide support to patients living with diabetes, respiratory disease and cardiovascular disease.

Manage workload effectively

Establish links with all practices, the PCN team and connecting with voluntary and services outside general Practice

Participate in the administrative and professional responsibilities of the practice and PCN Team

Ensure accurate notes on all consultations and treatments recorded in their System 1 notes

Recognise and work within own competence

Attend and participate in Practice and PCN meetings

Training & Development

Maintain continued education by attendance at courses as deemed essential for professional development

Participation in annual appraisal review

Maintaining a record of own personal and/or professional development

Governance

Contribute to the assessment of service needs

Manage and organise individual patient consultations

  • The post-holder will manage their own and others health & safety and infection control as defined in the SEL GP Group Health & Safety Policy, the SEL GP Group Health & Safety Manual, and the SEL GP Group Infection Control Policy and published procedures.
