Do you want to be part of a new, emerging service who are passionate about preventing hospital admissions, supporting early hospital discharges and delivering personalised care in the comfort of peoples homes? In keeping with the new developing transformation change to using virtual ward models, Sirona care & health are collaborating with UHBW & NBT NHS Trusts to deliver the NHS @ Home service to patients across BNSSG (Bristol, North Somerset & South Gloucestershire).

As a team we are blending medical innovation with bespoke care, bridging the gap between hospital and home. A virtual ward is a safe and efficient alternative to NHS bedded care that is enabled by technology and home visits. This innovative approach enables us to deliver safe, high quality and personalised care to people at home where they would rather be.

We know that people recover faster and better in their own environments and our dedicated teams of trained staff endeavour to support this. A hybrid model of virtual, telephone and face to face consultations can be delivered as required, allowing patients to recover in their own home with continued clinical support.

The Hub team currently provides support for Respiratory, Frailty, Heart Failure and Home IV patients, with the desire to develop and expand further pathways

Main duties of the job

Weare looking to recruit a Band 4 Nurse Associate withrelevant skills, experience and qualifications working with service users on our NHS@Home Pathways

Our Nurse Associates provide holistic, person-centred care and support for service users of all ages in a variety of settings. You will be bridging the gap between health care support and registered nurses, whilst working safely within cope of practice and competence at all times.

Key responsibilities include:

  • Undertaking holistic assessments
  • Implementing care programmes
  • Assessing and monitoring service users health needs

Base location: John Milton Clinic, Henbury

Service hours: 8am-8pm, 7 days per week including bank holidays. Your shifts will be allocated on a rota basis and will incorporate a variety of shifts within the above hours

Qualifications

Essential

  • Successful completion of relevant Foundation Degree modules / Diploma in Clinical Health and Social Care / diploma level 3 or equivalent.

Desirable

  • Able to carry out the relevant clinical competencies required of the post implement and evaluate care, using agreed protocols reporting adverse signs to registered professionals, specialist services, GPs or others
  • Contribute to the holistic assessment of a service user
  • Communicate in varied ways with colleagues, service users and other stakeholders
  • Able to use a variety of IT platforms i.e. EMIS; email
  • Demonsrate knowledge and unersstanding of Clinical Governance, Clinical Risk and to use Evidence Based Practice in order to provide optimum care to patients.
  • Participate in supervision and the induction of new members of staff and undertake an assessor course or equivalent, if appropraite
  • Experience of direct client care in a health or social care setting.
  • Experience of team working and working without direct supervision.
