Job summary
The post holder will work as part of the Children’s Community Nursing Team, in the delivery of support to families with children and young people who have additional health needs identified.
To work independently with delegated responsibilities for a defined patient caseload from the Children’s Community Nurses, in range of settings appropriate to the child and/or young person e.g. school, clinic or home.
To undertake planned assessments and clinical interventions following agreed care pathways and individual care plans, ensuring all deterioration or changes are reported to the appropriate Children’s Community Nurse/Clinician. E.g. Blood pressure monitoring, respiratory, cardiac, enteral and wound care.
Main duties of the job
Undertake systematic assessment and clinical care for identified children from the CCN caseload, following evidenced based clinical guidelines and pathways including blood pressure, oxygen saturation monitoring, cardiac reviews, blood sampling, routine gastrostomy and wound care.
Issue clinical equipment and reviews children’s progress following the CCN sleep study standard operating procedure.
To deliver care without direct supervision working together with patients, their parents and the multi-disciplinary team promoting self-care and reduce medicalisation of the their daily activities.
Prioritises their own workload and understands the boundaries of their practice.
Identifies changes in a child or young person’s condition, referring appropriately to parents/Lead Nurse or implementing escalation treatment plans as appropriate.
To contribute to health promotion activities relevant for the caseload e.g. infection control.
To liaise with the family and multi-disciplinary team when delivering support and seek appropriate supervision.
Contributes to maintaining a safe environment and the safe use of equipment and to report any unsafe environment/unsafe use of equipment to the Lead Nurse.
