A virtual ward is a safe and efficient alternative to NHS bedded carethat is enabled by technology and home visits. This innovative approach enablesus to deliver safe, high quality and personalised care to people at home where they would rather be.

We know that people recover faster andbetter in their own environments and our dedicated teams of trained staffendeavour to support this. A hybrid model of virtual, telephone and face toface consultations can be delivered as required, allowing patients to recoverin their own home with continued clinical support.

The Hub team currently provides supportfor Respiratory, Frailty, Heart Failure and Home IV patients, with the desireto develop and expand further pathways

Main duties of the job

The NHS @ Home Service (Virtual Wards) isa new and exciting development within the NHS and this post offers theopportunity to help establish and develop the BNSSG NHS @ Home Hub to supportboth early discharge of inpatients and admission avoidance. By using bespokesoftware, patients are monitored remotely using a variety of technology optionsand a daily patient symptom questionnaire.

As a SeniorClinician working in the NHS @Home service you will provide day to daymanagement of caseload and staffing, alongside the hands on elements of carryingout both home visits and virtual monitoring to support patients across BNSSG. Youwill lead on the daily co-ordination of staff, the monitoring and assessment ofpatients on our pathways, advising on care and treatment plans plus reportingchanges in patient conditions as required. You will provide direct linemanagement to a number of clinicians and support staff, offering them support,guidance and training where required.

Flexibility, willingness to learn and a’can do’ attitude will be essential in this exciting new seven-day service.This post is open to all nursing and AHP staff with relevant experience -training in the use of remote technology will be included within the inductionof the post.

Job responsibilities

  • Triage patients prior to acceptance into caseload; using multiple systems to support decision making.
  • Coordinate daily allocation of service tasks to other staff including administration to ensure that all tasks are completed and enabling safe, timely care
  • Lead the monitoring and assessment of patients in the @Home service, advising on care and treatment and reporting changes in patient condition as required in line with local protocols and guidance
  • Support colleagues within the @Home service to complete tasks efficiently and effectively and as directed by senior members of the virtual ward
