We have an exciting opportunity to join the East and North Herts Older Peoples Service in a specialist role to support discharges from mental health in-patient units.

Main duties of the job

Your role will be to work in a Multi-Disciplinary team to assess social care needs for individuals diagnosed with an Organic mental health condition. You will have good knowledge of the Care Act and relevant legislation including the Mental Capacity Act and Mental Health Act. You will attend Care Programme Approach (CPA) meetings for individuals and attend multi-disciplinary meetings on the wards.

We promote personalisation through our new assessment process focusing on the wellbeing of the individual and strength-based approach. Through care planning we strive for increased independence, choice and control by offering personal budgets to enable people to direct their own support.

We have bases at Farnham House, Stevenage and County Hall, Hertford however we encourage working flexibly with the benefit of a laptop and smart phone so that you have the option of working at home or at one of our touchdowns across the area.

The post requires you to be a Registered Social Worker and essential car user.
