JOB TITLE: Clinical Pharmacist
HOURS: 24hours per week over 3 days
REPORTS TO: The GP Partners clinical and professionalactivities

JOB PURPOSE
Provideexpertise in clinical medicines review and address public health and socialneeds of patients in the Practice
Reduceinappropriate polypharmacy and wasteful prescribing through clinical medicationreview
Reconcilemedicines following hospital discharge and work with patients and communitypharmacists to ensure patients receive the medicines they need post discharge.
Contributeto reductions in medicine related hospital admissions and readmissions bysupporting patients to get the best outcomes from their medicines andidentifying and addressing medicines related issues
Interfacewith community and hospital pharmacy colleagues and develop referral processesbetween primary care professionals including the promotion of the repeatdispensing service
Oversee andsupport the dispensary function of the Practice
Oversee therepeat prescribing reauthorisation process, including reviewing requests forrepeat prescriptions and medicines reaching review dates
Managepatients and health care professionals medicine queries
Increasequality and safety of prescribing through mechanisms such as audit and PDSA(PLAN DO STUDY ACT) cycles
Managepractice formularies to improve the quality, safety, and cost effectiveness ofprescribing
Implementdrug withdrawals and alerts e.g., MHRA, aimed at improving medicines safety
Work withprimary care professionals and patients to implement NICE and other evidence-basedguidelines
Work withCommissioning organisations to comply with local medicines optimisation schemesand projects
Support andhold clinics where medicines are the main interventions e.g., asthma clinics
Contributeto multi-morbidity reviews and multidisciplinary reviews
Providemedicines information and training to Practice healthcare professionals andadmin staff
Review dailyPathology results for patients on known medicines
Act as asource of medicines information for all the practice teams and patients (e.g.,around doses, side effects, adverse events, possible alternatives e.g., aroundout of stocks)
Ultimatelyto undertake minor ailments triage, dealing with minor ailments and triagingpatients appropriately
CORE RESPONSIBILITIES
Workingwithin the practice-based team to undertake medication reviews particularly in high-riskgroups such as:
-Frail elderly
-Polypharmacy
-Renal impairment
-Hepatic impairment
-Asthma, diabetes, hypertension and Rheumatoid and DMARD drug monitoring
-Substance misuse
-Patients on high-risk medicines
-STOPP/START identified patients
-Revolving door Hospital admissions
Managementsupport for the dispensary including profitability and stock control;negotiations with drug company representatives and wholesalers; andorganisational management including review of SOPS (Standard OperatingProcedures) and compliance issues
Ensure thePractice continues to participate in, and meet the requirements of, the localDSQS (Dispensing Services Quality Scheme)
Improveprescribing practice through educational support for all prescribers within thepractice
Lead onwhere changes in evidence require changes in prescribing across the patientpopulation, e.g., where a drug is withdrawn, or indications change
Liaise withcolleagues in community pharmacy to align support for medicines adherence suchas MUR (Medication Use Reviews) and NMS (New Medicine Service).
Supportimprovements in clinical care through practice-based audit and implementingchange
Prescribingadvice to prescribers in practice e.g., temporary non-availability of drugs
Ensurepatient safety when they are transferred between care providers throughreconciliation of prescribed medicines
Lead formedicines optimisation scheme compliance, including care home support;discharge reviews; medication reviews; and QIPP
Qualifications andTraining for the Role
NB: it is anticipated that the level ofqualification held may vary according to the level of position and thecomponents of the role being undertaken
Completionof an undergraduate degree in Pharmacy, and registration with the GeneralPharmaceutical Council
Minimum of 2years post graduate experience in pharmacy, as demonstrated within a practiceportfolio
Member of the RoyalPharmaceutical Society (RPS) Desirable
Member of or working towards(through foundation) RPS Faculty membership May hold or be working towards aprescribing qualification
May hold or be working towardsa postgraduate pharmacy qualification
Has anawareness of the breadth of common acute and long-term conditions that arelikely to be seen in a general medical practice
