Chelford, Handforth, Alderley Edge, Wilmslow (CHAW) PCN and CWP’s Children Young People and Families mental health services have worked together to enhance access to mental health support.

We are looking for an experienced Mental Health Worker who is interested in working as part of the GP Multidisciplinary Team in CHAW Primary Care Network.

This is part of an exciting new development to improve collaborative working across primary and secondary care, the role will contribute to the expansion of community mental health services as set out in the NHS Long Term Plan. This brand-new role has arisen as part of the transformation with our partner agencies to develop the interventions that can be offered at primary care level. The successful applicant will be part of a team of practitioners aligned to our Cheshire Well Being Hub working in identified GP practices across CHAW.

The role will include completing assessments, care planning and delivering interventions for patients registered at the GP practice who have mental health needs. The role will also involve a strong navigator element to ensure the provision of advice, education and support to children, young people or family members. You will also act as the interface between partner agencies to ensure effective transition to primary care and referral into secondary care settings when necessary.

The admin base is Hawthorn centre and Clinical Work covers – Chelford, Handforth, Alderley Edge, Wilmslow

Main duties of the job

You will also be actively involved in the interventions provided by the wider GP multidisciplinary team and provide expert advice and support to enable them to provide an effective service for children and young people with mental health needs including their physical health needs.

CHAW PCN has also invested in social prescribing and this is embedded in a team of Adult Social Prescribers and therefore the CYP Mental Health Worker role will have strong links into the community. There is an established, seamless path into this service, which can also help support the parents and families of young people. Our conversations were used to plan changes so that children and people in the Chelford, Handforth, Alderley Edge and Wilmslow area can get the help and support in the right place for them, when they need it, to stay mentally well.

Primary Care Networks (PCNs) are groups of GP practices working together in a local geographical area. CHAW Primary Care Network is a collaboration of the 6 GP Practices who collectively care for over 47,477 patients.

You will receive direct support and guidance from the GP’s about your workload but also be line managed and professionally supervised by the CWP Band 7 Cheshire Wellbeing Hub Lead. As the post is hosted via CWP you will also have access to training, education and support from CWP. This new post would suit individuals who are committed to collaborative working to improve outcomes for people with mental health issues.
