We are looking for an enthusiastic ward based pharmacy technician to join our rapidly expanding and friendly department at the Royal Wolverhampton NHS Trust.

You will receive excellent support and training from our senior pharmacy technicians and pharmacists.

Main duties of the job

We have a great opportunity for a Ward Based Medicines Management Technician to join our pharmacy team.

Whether you are already a band 5 looking for a different challenge or a band 4 technician with 2 years post qualification experience we would love to hear from you.

The post holder will primarily be working at ward level providing a pharmacy service to include:

  • Assessment of Patient’s Own Drugs (PODs)
  • Medicines Reconciliation
  • Supply
  • Discharge
  • Counselling

Training, supervising and coordinating the work of band 4 rotational pharmacy technicians, ATOs, students and support staff at ward level.

Leading the team of technicians on a rota basis, preparing weekly and daily rotas working closely with the senior technician and lead pharmacist.

A full and comprehensive training package will be provided.

There will also be a commitment to the dispensary to carry out the final accuracy checking of medication for a minimum of 2 hours per week, therefore a current and valid Accredited Pharmacy Checking (ACPT) certificate is required (consideration will be given to those candidates who are already in the process of gaining this qualification).

The post-holder will participate in our late night, weekend and bank holiday rotas.

Job responsibilities

To work on the wards to provide a Ward based Medicines Management pharmaceutical service to the Trust.

To complete a drug history and medicines reconciliation for each patient using at least 2 different resources and referring any ambiguous or discrepancies to the ward pharmacist for clarification / investigation.

To undertake locker checks according to SOPs and remove items no longer required from patient lockers (updating the Patient Medication Record (PMR) accordingly).

To assess Patients Own Drugs (PODs) for fitness for continuation of use. This includes fitness for intended use, expiry date, packaging, and storage.

To counsel the patient on current medication usage and on any changes additions and deletions to normal medication regime and establishing if Patient Has Own Supply at Home (POSH) in readiness for discharge.

To ensure at all times that enough stock has been ordered and will be made available for the patient’s treatment. This will include controlled drug stocks and discharge medication.

To liaise with the carer or relatives of the patient to obtain non formulary medication or arrange alternative supply to ensure continuation of patient care.

To liaise with patients GP, Nursing home when individual compliance aid boxes are required to ensure patient continuity of treatment.

To plan and prioritise workload of urgent and requested medicines to ensure continuation of care.

To participate in the return, recycling and destruction of medicines from wards and departments that are no longer needed.

To provide information and liaise with medical and nursing staff on the safe economical use of medicines, reviewing their stock needs on a regular basis.

To undertake collation of work-based statistics as required on aspects of the clinical service.

To undertake audits of service under the direction of the senior technician.

To undertake the receipt of prescriptions direct from patients, levying the relevant charges.

To receive, interpret, validate, dispense and issue routine and non-routine prescriptions for in-patients and out-patients, seeking advice from senior staff where appropriate.

To dispense and replenish controlled drugs completing the necessary legal documentation.

To dispense and record appropriate details, according to written protocols for unlicensed and named patient medicines within pharmacy.

To final check medicines for accuracy after completion of a checking technician qualification.

To take all appropriate action to ensure that patients receive the correct medication, with advice and counselling and that all directions associated with medicines are fully understood.
