We have a fantastic opportunity for a dynamic & enthusiastic Band 6 nurse who has vision, passion & drive for education & the development of others, to join our District Nursing Service as a Practice Facilitator.
The successful candidate will be responsible for co-ordinating & delivering a comprehensive service specific learning experience.
The role is beneficial to improve clinical outcomes, increase patient satisfaction & contribute to quality health care provision. You will support the nursing team in the delivery of high standards of patient focused, evidenced-based care. You will have the opportunity to work with the Programme Management Office (PMO) to implement & educate new initiatives & guidelines.
We are expanding the practice development team to support new starters & assist in upskilling current staff.
The role is designed to influence the quality of the clinical learning experience by providing support through education, training, coaching & facilitation. Most of the working time will be spent working alongside the staff within the clinical areas, to enable clinical guidance to staff development, observe practice & assess staff competency in clinical nursing skills but to also ensure nurses are adhering to best practice (N.M.C. Code of Practice & professional standards & guidance).
You should have drive, demonstrate innovative thinking & ideally have previous experience of implementing/embedding change & quality improvement projects.
Main duties of the job
You will be required to plan & deliver regular training sessions to the Community Nursing teams. You will possess advanced clinical assessment skills.
You will need to have in depth knowledge & skills required for the role of a community nurse with previous experience in education & staff development being desirable.
You will have a comprehensive knowledge & understanding of government directive & National & Local Policies, Evidence Based Practice & Best Practice statements.
The Practice Facilitator role is linked directly to the development of clinical and leadership competencies for staff groups across the District Nurse Teams.
This person will have all the clinical competencies in place to be able to develop the skill and knowledge of the staff groups. The post holder provides support to the clinical nursing staff to underpin the principles of evidence-based practice and quality in professional practice development and service specific education.
The post holder leads on the implementation of clinical qualities and standards, providing a visible, accessible and authoritative presence in the community settings. The post holder will be working across organisations and communicating with a range of professional, patients and their families.
The post holder will provide expert advice and clinical support whilst promoting the effective and efficient delivery of high-quality service within the local community.
