The University Hospitals Sussex Foundation NHS Trust is delighted to be working in partnership with Teenage Cancer Trust to host a 1 WTE dedicated Teenage Cancer Trust Teenage & Young Adult Cancer Clinical Nurse Specialist to support young people (16 -24 years old) diagnosed with cancer in Brighton. This is a very exciting opportunity to become part of a dynamic and expert team and lead the way in improving provision of TYA cancer care through collaborative working and shared commitment to young people with Cancer.
Main duties of the job
The post holder will work alongside the TYA lead clinician & nurses, based at the Royal Sussex County Hospital / Princess Royal Hospital, to establish close links within cancer services. As a local champion for TYA cancer care, you will be required to work as part of the wider regional South Thames TYA Service team and the Principal Treatment Centre (Royal Marsden Hospital). The post holder will be required to work autonomously and exercise a high level of clinical expertise when identifying and caring for the specific needs of TYA patients and their carers. You will need excellent interpersonal skills, senior nursing experience, knowledge of TYA cancers and the ability to work sensitively and collaboratively with diverse teams. By working alongside the TYA team and Site-Specific Teams you will promote a cohesive approach to developing TYA care within the region, in line with national guidelines.

Work closely with the TYA team and the site specific multi-disciplinary teams locally and at the Principal Treatment Centre to ensure alignment of goals and priorities between national and local modernisation initiatives relative to TYA Cancer services. Provide comprehensive highly specialist care and advice for teenagers and young adults with cancer (both to the patient and the specialist teams) Prepare, participate and contribute to the TYA Multi-Disciplinary meeting at University Hospitals Sussex Foundations NHS Trust ( RSCH/PRH sites) and ensure that all newly diagnosed and relapsed TYA patients are discussed at the Principal Treatment Centre TYA MDT and attend this on a weekly basis Ensure TYA cancer patients have regular holistic assessments using theIAMs portal throughout their journey to be presented at the local and Principal Treatment Centre TYA MDT. Provide patient core contact for the TYA service through media available to the patient e.g., face to face, text, telephone, home visit (if necessary),website, e-mails, written information. Promote a culture that supports self-management of the ongoingconsequences of treatment, including completion of End of Treatmentsummaries and care plans for patients that are being key worked Actively ensure that the voice and needs of TYA patients and carers are at the hear of all service improvement and have representation within the Trust patient and public involvement and engagement strategy
