Main duties of the job

The post holder will be expected to:

Support the delivery ofclinical pharmacy services such as medication reviews and medicinesreconciliation

Increase the quality and safety ofprescribing through medicine safety initiatives andproject work

Deal withpatient and healthcare professional medication-related queries

We are seeking a pharmacy technician to complement the teamand to play a key role in delivering the PCN agenda.

Whilst being part of the PCN pharmacy team you will be based in the Northand West Neighbourhood (Chatsworth Road Medical Centre, Newbold Surgery andWhittington Moor Surgery) helping to deliver the PCN Direct Enhanced Service(DES).

Key areas of Focus

Clinical

-Undertake patient facing andpatient supporting roles to ensure effective medicines use, through shareddecision-making conversations with patients.

-Carry out medicines optimisationtasks including effective medicine administration (e.g. checking inhalertechnique), supporting medication reviews and medicines reconciliation. Whererequired, utilise consultation skills to work in partnership with patients toensure they use their medicines effectively.

-As determined by the PCN, supportmedication reviews and medicines reconciliation for new care home patients andsynchronising medicines for patient transfers between care settings, linkingwith local community pharmacies

-Support the clinical pharmacist todeliver structured medication reviews (SMRs) i.e. organise necessary monitoringtests prior to SMR

-Provide expertise to address boththe public health and social care needs of patients, including lifestyleadvice, service information, and help in tackling local health inequalities.

-Manage shared care protocols andliaise with clinical pharmacists for more complex patients.

-Supportinitiatives for antimicrobial stewardship to reduce inappropriate antibioticprescribing locally.

Technical

-Support the PCN multi-disciplinaryteam to ensure efficient medicines optimisation processes are being followed.

-Implement efficient orderingprocesses and reduce medication wastage.

– Provide training and support on the legal, safe and securehandling of medicines, including the implementation of the ElectronicPrescription Service (EPS).

– Promotion of Electronic Repeat Dispensing (eRD) and onlineordering

-Develop relationships with otherpharmacy professionals and members of the multidisciplinary team to supportintegration across health and social care including primary care, communitypharmacy, secondary care and mental health.

-Support practice reception teamsin streaming general prescription requests, so as to allow GPs and clinicalpharmacists to review the more clinically complex requests.

-Support the implementation ofnational prescribing policies and guidance within GP practices, care homes andother primary care settings.

-Support the PCN to deliver on QIPPagenda, QOF and locally commissioned enhanced services

-Support thePCN in reviewing and developing practice policies for CQC requirements

All PharmacyTechnicians must work within their competencies, and Technicians should ensurethat professional indemnity cover via GP practices and PCN is adequate fortheir role
