Job summary
Working as a Social Worker or Senior Social Worker you will be part of a supportive and friendly team working with other social/health care professionals to improve the wellbeing of older adults.
You will need to demonstrate a sound understanding of how to apply a strengths-based approach to practice, whilst being person centred and able to promote safe and effective social care services in accordance with the Care Act 2014. You will assess service users in a variety of settings, including virtual platforms.
Main duties of the job
Ideally, we are looking for people who can demonstrate excellent communication skills and the ability to build and maintain positive relationships with their colleagues, other professionals and the people who use our services.
As a Social Worker, you will need good organisational skills (including the ability to balance competing workload demands). You will either be working on site at Basingstoke and North Hampshire Hospital, or completing face to face assessments in community settings .

Provide Supervision and Practice Leadership, in line with Professional Accountability
- Provide supervision for Case workers and Senior Case workers.
- Give practice advice to both Case workers and Senior Case workers on complex cases.
- Make professional decisions on cases and make recommendations to team managers about risk.
- Triage safeguarding referrals from emergency services and make decisions.
Respond to Phone and Online Queries
- Answer phones and deal with customer queries in line with strengths based practice.
- Triage telephone and online referrals.
- Identify risk and escalate safeguarding concerns as necessary.
- Write concise reports on customer’s files to effectively communicate to front line staff such as Social Workers.
- Deal with online forms.
- Answer webchat questions.
- Arrange appointments for clients to have online assessments with social workers.
- Conduct carers assessments.
- Carry out wellbeing assessments to identify client’s level of needs and support available.
- Follow up calls to confirm clients queries have been resolved.
- Record Compliments and complaints as per HCC policies.
- Be available to the customer at the correct shift times, unless agreed otherwise.
Maintain up to Date Knowledge and Skill Levels
- Attend all training to ensure knowledge is kept up to date and take responsibility to pro-actively keep self-informed of relevant policies and legislation.
- Take part in The Quality Assurance process and take ownership for improving skills/knowledge from the feedback.
- Develop and maintain specialist knowledge in relation to needs and resources in order to support Adult Social Work, Mental Health, Learning Disability, Sensory Service and Occupational Therapy.
