Principal duties and responsibilities

  • To work with individuals and groups of carers to enable them to feel supported and less isolated
  • To provide a focal point for information, advice and support about local services for carers
  • To support carers of disabled children and young people and during periods of crisis.
  • To raise awareness with health professionals about the needs of carers
  • To signpost carers to appropriate Carer Support Groups and Carer Forums across the City.
  • To assist the Team Manager in the identification and development of new carer services, in line with the long-term impairment strategy and long-term condition strategy
  • To work with other services inn offering practical support to carers including assessments and benefits advice.
  • To work with carers identified through GPs or health professionals.
  • To identify information appropriate to meet carers needs in a format that allows access by people with different forms of impairment and by those where first language is not English.
  • To ensure that services for carers are culturally appropriate.
  • To arrange information and training/education sessions to provide carers with the skills and knowledge they need (eg, understanding medication, continence, lifting and handling, challenging behaviour, first aid, hearing voices etc)
  • To enable carers to express their needs and put their views forward in various ways such as Carer task groups
  • To maintain an up-to-date knowledge of policies and initiatives including the national and local carers’ strategy to inform the development of good practice locally.
  • To ensure that records are maintained of activities undertaken to support carers and their effectiveness to enable the service to be monitored and evaluated.
  • To undertake and record separate Carer Assessment and where appropriate Support Plans.
  • To ensure compliance with the Council’s Equal Opportunities Policy, Health and Safety Policy and the Data Protection Act 1998 (and subsequent amendments).
  • To undertake such duties as are consistent with the job purpose and roll.
  • To act in accordance with the Council’s Constitution and other Codes of Conduct.
  • To participate in staff development, appraisal and training as appropriate, including continuous professional development.
  • To comply with the Council’s agreed policies and procedures including but not limited to Health and Safety, Equal Opportunities Polices, the Data Protection act, Freedom of information Act, Financial Management Regulations and other relevant Council and Government Regulations, Directives and City-wide priorities.
  • To undertake any other tasks, duties and responsibilities as directed and appropriate to the grade and role of the post subject to any reasonable adjustments under the Equality Act 2010.
  • To participate in the wider development of the service and contribute to service improvement as required.
