An excellent new opportunityhas arisen for an enthusiastic, experienced Specialist Stroke Nurse or AlliedHealth Professional to join our community stroke team to work 37.5hrs over 5days (Mon-Fri) – but willing to discuss flexibility of this. This will be for afixed 6-month period, but with the possibility of permanent recruitment.

The ability touse your initiative, work proactively, demonstrate excellent communication andinfluencing skills, manage and prioritise your own workload and respondflexibly to changing demands are essential requirements of the post.

You will be partof the Countywide Community Interdisciplinary Stroke Service which is currentlydivided into two teams: North and South. The whole team has a strong work ethicbut are extremely flexible and supportive in the balance of work and home life.

You willpredominantly work round the county of Worcestershire using both bases butthere will also be a requirement to work from home for which you will beprovided with the necessary equipment to do so.

Therole will provide you with variety and no two days are the same. You will haveautonomy to balance and achieve the demands of the role so will need todemonstrate abilities in being responsive and flexible.

Main duties of the job

Day to day, you will be undertaking comprehensiveassessments of stroke patients who are living in nursing/residential care homesor whom have a package of care at home. These are some of our most complex andvulnerable stroke patients that we feel passionate about in receiving the rightspecialist support.

You will undertake autonomous individualised assessments toensure that their care needs are being met and assessing whether quality oflife could be improved in supporting with things like seating, positioning,pain management, spasticity management and psychological care. It is also expectedthat you will offer support and education to carers/families.

You will receive support from the wider team and beliaising with other health care professionals and third-party organisations inmaking onward referrals.
