Salary: £36,298 -£41,496 per annum (pro rata) plus a local weighting allowance of £1,002 per annum & shift allowance is also payable.
Post level: 7.
Directorate: People (Adults).
Hours of work: Full time 37 Hours, working on a shift pattern with a requirement for some evening and weekend working.
Location: The Pines, Forest Close, Wexham, SL2 4FJ.
An enhanced DBS is required for this role.

As we have launched a new Reablement and Independence Service, we have also agreed new working patterns and you must be prepared to work as per an agreed shift schedule, including working some evenings and weekends. Your main accountabilities will be
To work as a member of a community based multidisciplinary team.
Use Occupational therapy expertise to assess, implement, guide care plans for service users and carers, enabling them to achieve maximum independence in their own homes or residential settings.
Provide a broad range of support to clients who have physical or mental health problems, learning difficulties, dependencies or who are elderly.
Help to develop and/or maintain resident’s independence using a strengths based approach.
Requirement to work on a shift basis including evenings and weekends.
You will need to have
Ability to manage autonomously a clinical caseload of clients at a specialist level, assessing and maintaining clinical records.
To set SMART goals to ensure treatment is effective, and to improve client flow through the service.
To work collaboratively with health and social care community and acute colleagues.
To work with other members of the MDT to ensure effective assessment, treatment, management of care pathways and transfers of care / discharges.
Excellent time management skills. Including the ability to work to deadlines and in stressful situations.
Demonstrate positive attitudes and behaviours, taking a strength based approach to assessments.
Experience of working with people of diverse cultural and religious communities.
MSC/ Bsc/Diploma in Occupational therapy.
State registration (HCPC & College of OT).
Full driving licence with access to a car on a daily basis.
