A unique and exciting Band 7 opportunity has come available in Sussex Community NHS Trust (SCFT) to work alongside the Community Respiratory services to lead the development, set up and provision of a Trust wide Pulmonary Rehabilitation (PR) Hub.

We are looking for an experienced Nurse or Physiotherapist to clinically lead the transformation of PR services across the SCFT footprint.

Respiratory and PR experience is essential in order to take a lead role in the design and establishment of effective PR care pathways to ensure safe and effective services and optimum use of all available resources.

You will be based at Brighton General Hospital but will be expected to travel across the SCFT footprint working directly with the community respiratory teams.

Main duties of the job

Take a lead role in the design and establishment of effective PR care pathways to ensure safe and effective services and optimum use of all available resource. You will need to flexibly plan and organise own work to ensure a balance between transformation, management and clinical duties.

To provide leadership, challenge the status quo and work collaboratively with professional colleagues and other health and social care organisations, promoting collaboration and service development.

As part of your role is ensuring our service users receive excellent care, you will initiate and participate in evidence-based projects and audits, to improve both immediate and future quality of patient’s care.

In this role, you need to be an excellent communicator, enthusiastic, flexible and able to work autonomously using strong clinical and reasoning skills. You will also be passionate about working creatively to bring about innovative service development.

    The post holder will be the clinical lead for the transformation of Pulmonary Rehabilitation services across SCFT. You will be required to lead the development, set up and provision of a Trust wide Pulmonary Rehabilitation (PR) Hub, this will include a centralised triage function and support delivery of PR with the community respiratory teams across the SCFT footprint.
