The post holder will hold an active caseload of complex individuals with a learning disability. You will implement episodes of care around early intervention, prevention and time limited pieces of work.

The post holder will follow the nursing process from referral, assessment, care planning, intervention, evaluation and closure. The post holder will implement effective interventions which are research led and person Centred. You will attend professional and multidisciplinary meetings. You will carry out appropriate assessments as needed.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

  • Broad range of experience working in health & social care settings
  • Knowledge

    Essential criteria

  • Demonstrate a good understanding of Learning Disability, including Mental Health, Forensics, Challenging Behaviour and PLD/Comples Health.-Awarenessof current developments within a clinical setting.
  • Completed training in mentoring & Assessing student nurses
  • Desirable criteria

  • Awareness of current developments within – Learning Doisability services
  • Understanding of clinical governance
