This contract is for a minimum of 6 months and offers a competitive rate ranging from £25 – £30 per hour, depending on experience. You will be working with a top time basis in the working within the Hospital Discharge team.

Responsibilities:

  • Working with adults and older people
  • Managing your own caseload
  • Carrying out assessments and Reviews
  • Developing Care plans
  • Report writing

    • Requirements:

  • Experience working in a Hospital Discharge role, supporting adult services
  • Be SSSC registered
  • Full or part time hours considered
  • Adult protection services preferred
