We are looking for a dynamic andforward thinking Senior Clinical Pharmacist with a specialist interest in caringfor frailty patients, to join the CDDFT team.

The post will be based at University Hospital NorthDurham but the post holder will be required to visit the other main sites inorder to deliver key objectives. The role is a fulltime fixed term contractwhilst the current post holder is on maternity leave for 12 months. However wewill consider flexible and part time working hours.

Main duties of the job

This clinical role provides excellent opportunities todevelop specialist knowledge and work closely with the medical and frailityteams to provide a meaningful clinical input into the care of patients. There are well established, good workingrelationships between pharmacy and the frailty team and a key part of the rolewill be to continue to work closely with the MDT to deliver the governanceagenda, teaching, safety improvements, and further develop the role. To achievethis we will support the successful candidate with mentoring and developmentopportunities including independent prescribing, RPS portfolio development andother further education opportunities. Applicants need to be able to workautonomously, be a team player whilst motivating others.

We are committed to the principle of pharmacistprescribing and all appropriately qualified and experienced pharmacists areencouraged to complete an independent prescribing course and to practice as anon-medical prescriber. The post will provide leadership and expertise to theother pharmacists and technicians within the Trust and will support the LeadPharmacists in moving the Trust pharmacy strategy forward.

Lead and manage a team of pharmacy staff and be responsiblefor managing the delivery of safe, effective and efficient pharmacy andmedicines management services to a defined group of patients and service users.
