We are proud to offer a Hospice at Home service which is part of our Community Team. This compassionate and experienced team of nurses and healthcare assistants remain with someone who is dying in their own home, providing direct hands on care and support between the hours of 10pm and 7am. The team works in partnership with the wider health system, particularly District Nurses and GPs, the wider Hospice team, and other specialists. We would love to grow our night time team to be able to offer this vital specialist care to more patients, therefore, we are looking for more Healthcare Assistants to join our Hospice at Home department in North Cambridgeshire.

Main duties of the job

In this role you will provide safe, effective, and responsive nursing care that meets the physical, spiritual, emotional and social needs of patients and their families/carers. You will work directly and alone with patients and families in their own homes.

Colleagues working within our Hospice at Home (HaH) team cover a wide geographical area, from Tydd St Giles in the North of the county down to Saffron Walden in the South, and everywhere in between. Although allocated to either the Cambridge (South team) or Wisbech (north team) base, colleagues work flexibly as one team in order to meet patient needs wherever they may be. For this reason, we are pleased to offer mileage expense rates in excess of HMRC as well as a lease car option. Whilst every effort is made to allocate patients close to where you live, this is not always possible we ask all of our colleagues to be flexible and attend patients wherever they are located.

Night time enhancements:

  • Time plus 36% – All time on Saturday (midnight to midnight) and any week day after 8 pm and before 6 am
  • Time plus 72% – All time on Sundays and Public Holidays (midnight to midnight)

Main Duties and Responsibilities:

  • At all times actively promote patient and family/carer involvement in care
  • Under the direction of the district nurse and on call nurse, in accordance with written guidance and procedures, undertake agreed ongoing clinical activities with designated patients. These may include:

Delivering, monitoringand documenting the patients progress in the District Nursing notes andelectronically onto S1

Administeringmedicines as per Marr Chart via oral or topical route to designated patientsfor symptom management (this includes administration of controlled drugsorally)

Provide personalcare, pressure area care, mouth care etc.

Providepsychological / emotional support for the patient and family.

Use mobile workinglaptop to report handovers to the Hospice at Home Clinical Nurse Specialist thefollowing morning

Verbally reporthandover to the Clinical Nurse Specialist in Hospice at Home (when mobiledevices are not working)

Have the abilityto liaise with a multi-disciplinary team

Discuss the roleof Chaplain and Complementary Therapist and make recommendations for referralsto Clinical Nurse Specialist
