SWAGGA Primary Care Network (PCN) is a dynamic collaborationbetween sixteen practices across South Liverpool. We have a vision to improvepatient care, tackle inequalities and provide outstanding support for ourmember practices. We plan to make a lasting impact upon the communities weserve and are developing ambitious plans for the future. As a result, the PCN intendsto increase its management capacity and aims to appoint an Assistant Manager totake forward our strategic priorities and provide support across a range ofactivities to the current Network Manager.

Our combined patient population is approximately 101,000. Our networkworks very closely with our community team and other local Healthcareproviders. The network adopts new methods of working via an extended clinicalworkforce which includes Salaried GP’s, Advanced Clinical Practitioners,Clinical Pharmacists, Practice Nurses, Social Prescribers, and First ContactPhysiotherapists. We are keen to add additional management capacity to thenetwork as we further develop our teams. The network is a busy organisation,and the role will be varied and wide ranging.

Main duties of the job

The successful applicant will support the current NetworkManager, providing input and advicein relation to key decisions. To do this successfully, the post holderwill be up to date with regards to key policy developments, the PCN DES and wider NHSE/ICB initiatives.

Key Responsibilities and Duties:

a) Support the PCN Manager and Clinical Directorsto ensure the smooth running of the Network.

b) Maintain relationships across all memberPractices.

c) Point of contact for Practice Managers regardingdelivery of Network projects.

d) Work as a member of the PCN providingsupport to other team members.

e)Co-ordinate HR Management of PCNstaff, including managing appraisals and PDPs. Support recruitment for ARRs effectiverecruitment and retention strategies to develop systems andprocesses to manage the HR requirements for all PCN posts.

f) Manage the workflow processes in relation to theabove.

g)Build relationships and identifyopportunities to work with wider system partners.

h) Play a key role in the delivery ofhigh-quality primary health care within SWAGGA.

i) Ensure the PCNs compliance with the GDPR andFOI along with other relevant legislation and professional informationmanagement standards such as GMC/BMA and LMC guidance

j)Develop and maintain all PCN databases andpolicies.

k) Carry out any other duties as may reasonably berequired by the Network Manager, Clinical Director, PCN Board and its members.
