An exciting opportunityhas arisen to join a newly commissioned service as a Case Worker, Band 6,(RNLD, RMN, Social Worker, OT, Psychologist) in the Herefordshire IntensiveSupport Team based at Liverpool Court, Commercial Road, Hereford.

The purpose of the IntensiveSupport Team is to improve health and care services for those who have alearning disability and/or autism with mental health needs and forensic needswho may be at risk of care breaking down to continue to live in the community,with the right support, close to home.

The successfulcandidate will specialise in the complex assessment, management and treatmentof needs for those who have Learning Disabilities and or autism, offeringbespoke pathways to recovery within the community.

We are looking for a passionateand motivated individual who has sound knowledge and experience ofperson-centred care, positive behavioural plans, risk assessments/riskmanagement plans and relapse plans that incorporate low arousal approaches topromote least restrictive practice.

The successfulcandidate will ideally have previous experience and knowledge of mental healthneeds amongst people with a complexity of LD/A related needs and of the mentalhealth act, mental capacity and criminal justice legislation frameworks isdesired.

You will be joining asmall but committed team and will receive regular supervision, training andsupport within this role

Main duties of the job

Practice and maintain high levels ofTrust policy/ standards, understanding and adhering to local, regionals andnational policies and procedures and guidelines ensuring innovative and safepractice.

Ensuring the process of referralassessment planning intervention monitoring and evaluation of care iseffectively delivered with clear quality outcomes.

Work proactively with service usersidentified as being at risk of placement breakdown or hospital admission.

Participate in developing monitoring andauditing the standards of practice are researched evidence based in line withclinical governance.

Promote peoples equality diversity andrights.

Contribute to the development andimplementation of the learning disability service annual work plan.

Frequent exposure to distressing oremotional circumstances.

Input and retrieve activitydata into Care notes on a daily basis as required. Uses trust approved electronicsystems as appropriate, including ESR, Ulysses.
  • Activelymanage an effective caseload, maintaining regular visits to service users.
  • Facilitate and teach effective systems to carers to enable thetransfer of skills required for the management of service users behavioural andcomplex needs.
  • Attendteam and multi-professional meetings, as appropriate.
