We are currently recruiting for a Blood Pressure Care Navigator to be part of the Citywide Care Navigation Team.

We work with a strengths-based approach and champion independence, joined up care and encourage bigger and wider conversations.

The post holder will act as a community liaison and coordinator to work alongside Integrated Neighbourhood Teams, Nuffield Healthcare and GP practice/hospitals across Manchester to improve communication and collaboration including supportive signposting people in the community. This will ensure access to the right support, at the right time, in the right location. The service has a large community-based focus and links directly with people within their own homes and communities, collaborating with our stakeholders within Nuffield Health to improve long term health outcomes for people with high blood pressure. Identifying the warning signs early to prevent potential future deterioration in health.

Main duties of the job

The post holder will work alongside members of the multi-disciplinary team to enhance communication and connection leading to leading to more effective citizen care with a focus on keeping people safe, well and independent in the community. The post holder will also be able to manage their own diary and workload autonomously.

The successful candidates will have a key role in navigating Health, Social Care and Voluntary Community Social Enterprise (VCSE) sector systems. They will act as a vital link for community, hospital and primary care teams to ensure care is effectively managed in a community setting.

The successful candidates will require confidence and diversity with a range of computer skills and will be excellent communicators. Every day is different in Care Navigation and you’ll need to think on your feet and be prepared for any situation!
