Are you a Geriatrician or a GP with a specialist interest in Frailty who is looking for a new way of working? Whether you live in Land’s End or John O’Groats, we are looking for a Consultant Geriatrician / Frailty GP to be part of our Virtual Frailty Wards across the Mid and South Essex (MSE) Integrated Care System (ICS). The Frailty Virtual Wards aim to deliver high quality subacute care, in people’s own homes, within the MSE footprint. There are currently four Virtual Wards across the ICS delivered by our Community Collaborative. You will join a diverse multidisciplinary workforce who want to deliver the best care for frail older people in the community.

Main duties of the job

These innovative roles are designed to be flexible and work for you. With the right technology, provided by us, you will lead the daily Virtual Ward rounds and Multidisciplinary Team Meetings remotely or at a specific location, as you prefer. You will provide senior clinical oversight for our patients on the Virtual Ward, including advice and guidance, complex case management, prescribing and virtual patient consultations, as appropriate. You will cover the Frailty Consultant Hotline to support management of frail older people in the community and prevent unnecessary hospital admissions.

You will work closely with the frailty teams at the Mid and South Essex NHS Foundation Trust and be ‘hosted’ by the Community Collaborative within the MSE ICS. You will have remote access to education and training including Grand Rounds.

The Job Role:

These new Consultant / GP posts will support the expansion of our innovative Frailty Virtual Ward in the Mid and South Essex ICS. The role will work across the three community providers that deliver the Frailty Virtual Ward.

Clinical Governance and Departmental Meetings

The department has monthly clinical governance meetings which are fed into the Directorate group. There are regular Consultant business meetings to ensure adequate planning for our department.

Clinical Governance:

To provide clinical services in line with the Provides clinical governance arrangements which is designed to ensure that agreed quality standards are achieved. These requirements include:- Audit:

To undertake audit of clinical practice within the department to ensure that current standards and evidence-based practice are applied.

Continuing Professional Development:

All Consultants are required to maintain and develop their clinical skills. Individual training and development needs will be identified through an appraisal process. The Trust supports the requirements for continuing professional development (CPD) and is committed to providing time and financial support for these activities.

Risk Management :

To work within Provides clinical risk management policies and in particular to participate as appropriate in clinical incident reporting.

Appraisal and Development:

All Consultants / GPs with a Special Interest in Frailty are required to participate in our appraisal process. This is conducted by a trained, nominated appraiser. It is expected that the successful candidate will comply with the appraisal process. Yearly appraisals are a mandatory requirement as a part of the GMC Revalidation cycle for all medical staff in the NHS.

Teaching and Training Activities:

There is an expectation that you will be involved in the development of junior medical staff and staff / colleagues in the service.
