Salary: Marie Curie Clinical Pay Scale, alignedwithAFC BAND6 £33,706- £40,588pro rata per week

Contract Type: Permanent

Based: Carmarthenshire

In this key role, supported by the Clinical Nurse Manager, youll deliver day to day support to care homes across Carmarthenshire. As part of your role you will deliver direct support to patients at end of life. This will include supporting care home staff with training and development, advanced care planning awareness sessions and supporting people through grief and bereavement.

A confident communicator and calm decision maker, youll need experience and knowledge in palliative care and community services, together with well-developed assessment and planning skills. The ability to prioritise your work, whilst juggling a variety of changing demands, is vital. You will deputise for the Clinical Nurse Manager in their absence

The role is largely based inour Dafen office, although you must be willing to travel to various venues acrossWalesto meet with nursing teams and attend relevant End of Life Care meetings. The ability to drive with access to a vehicle insured for business use is therefore essential.

  • An understanding of specific needs of patients approaching end of life and in the terminal phase.
