Working Hours – 37.5 hours per week

This post is open to registered dietitians or those eligible to register with the HCPC as a dietitian within the next 6 months. 

You will work as a project dietitian within the diabetes service. The project that you will be involved in aims tosupport people who are newly diagnosed with diabetes to manage their own health by further expanding provision of structured education and digital self-management support tools. 

Main duties of the job

The role aims to look for innovative ways to increase uptake and access to diabetes structured education by implementing service improvements that include: 

  • Using data collection to inform decisions 
  • Investigating the barriers to patients accessing education and improving uptake.
  • Improving access for hard-to-reach groups
  • There will also be patient contact as part of this role that will include: 

  • delivery of diabetes structured education 
  • one to one appointments 
  • telephone calls to explore barriers to accessing education 
  • Travel between sites will be required. This will include GP practices within the Halton and Cheshire West footprint. There will be scope for some remote working.
