The post holder will be a key member of our multidisciplinary teams including a Consultant Geriatrician, Advanced Nurse Practitioners, Registered General Nurses, Paramedic Practitioner, Healthcare Assistants and Therapists to provide an urgent community response service to patients who are experiencing a health crisis with the aim to support and provide virtual ongoing management to them in their own homes (including care homes) rather than being admitted to hospital.

You will be a team player and work collaboratively with staff from all disciplines whilst maintaining high professional standards. You will ensure good communication, and effective processes and procedures are maintained across the post holder will work per week, 7 days a week, between the hours of 8am – 8pm, which will include weekends.

Main duties of the job

o To practice at an advanced level of nursing, working as an autonomous practitioner within the multidisciplinary team including admitting to and discharging patients from the Urgent Community Response Service in collaboration with the Community Geriatrician and GP.

o As an experienced nurse practitioner, you will take responsibility for the clinical assessment, diagnosis, treatment and management of complex patients considering local and national guidelines.

o The postholder is expected to liaise effectively and professionally with all levels of staff, external organisations and the public, dealing with confidential information in an appropriate manner.

o To provide leadership and line management to the Band 6 nurses and the Band 3 & 4 healthcare/multi-therapy assistants.

o Ensure accurate, contemporaneous documentation and care plans are maintained accordance with the NMC, information governance and local / national guidance.

o Work collaboratively with colleagues and develop nursing and care delivery policies, identifying how policy will impact on practice and service delivery.

o Work flexibly in response to service need across our multiple sites
