As a member of the Sector Leadership Team, the Systems PartnershipTransformation Manager (SPTM) is accountable for developing, building and maintaining productive and highly effective relationships with internal/external key stakeholders to improve service delivery, efficiencies and outcomes for patients

Main duties of the job

The intent of this role is to improve the patient flow for patients that are both conveyed to hospital and referred elsewhere within the community. Through creation of new ACPs and initiatives the PPM will reduce the number of patients that require conveyance to the ED, for those that do require conveyance the PPM will be responsible for ensuring the right procedures are in place to ensure a timely handover which will reduce the amount of lost ambulance time as a result of hospital delays. The post holder will also be accountable forwork streams and formal targets that will be set by the local ADO.
