JOB SUMMARYThe post holder will be part of a team of pharmacy staff providing expert pharmaceutical care to patients within Mid and South Essex.

MAIN DUTIES & RESPONSIBILITIESTo assist the Principal Clinical Pharmacists and other Pharmacy Managers in the provision of a comprehensive pharmacy service.This includes providing clinical and technical services, medicines information / management, dispensary support, giving advice to medical and nursing staff, counselling patients, including emergency services out -of-hours according to an agreed, rostered on-call commitment. Weekend services will also be needed.

Main duties of the job

Clinical Dutieso Ensure that prescriptions are dispensed accurately and legally in accordance with law and guidance of the General Pharmaceutical Council.o Provide clinical pharmacy services to specified wards within the Trust. The post holder will be encouraged to develop new models for delivering pharmaceutical care to specified groups of patientso Provide professional and clinical advice to pharmacy colleagues, nursing and medical staff and patientso Reporting to directorate pharmacists on medicines management issues relating to their directorateso Undertake medicines reconciliation at admission and discharge for all patientso To undertake medication reviews using knowledge and experience.o To review prescribed medications and assess the safety of these based upon patients clinical condition and other factorso To be actively involved in clinical governance aspects of the pharmaceutical service.o Maintains personal registration with the General Pharmaceutical Council by undertaking CPD.o To assist in the maintenance of other pharmacy services as required and to participate in Saturday/Sunday, Twilight, Bank Holiday and On-Call working as part of a roster
