Living Independently for Everyone

Rapid Response/Virtual Ward Matron

(hospital based)

Our vision is for the people of Croydon to experience well-co-ordinated care and support in the most appropriate setting, which is truly person-centred and helps them to maintain their independence.

As demand rises with increasing health needs and advances in clinical care, we are responding to the challenges whilst continuing to deliver the highest quality of care. We are developing the services that we currently provide to reduce hospital admissions and to facilitate earlier discharge from hospital through the use of technology, ensuring patients have the safest best care that is delivered in their home environment via the Croydon Virtual Ward.

The Virtual Ward service available 7 days a week.

Thepost holder will require advanced assessment skills and have the ability to identify and facilitate early discharge for patients via the Virtual Ward. The candidates will work closely with the local Remote Monitoring Hub and community teams and ensure clear care plans are in place on discharge.

They will need to be motivated, innovative with strong leadership and communication skills, to support the development of the Virtual Ward.

For further information or an informal discussion please contact: Operations Manager, Aisling Vaugh on 0208 274 6445.
