Main duties of the job

The post holder will be jointly responsible for

Assessing the training needs in terms ofend of life and palliative care education for Calderdale

The development and delivery of aprogramme of end of life care education for clinical teams within theCalderdale community settings and the Hospice to strengthen end of life carefor patients and families. This willinclude GP surgeries, district nurse teams and care homes

Provision of key impact data, includingsessions delivered, number of attendees and feedback from attendees

Line management of the Educators for Palliative Care Education in CareHomes

On a rotational basis to act as Chairand be a member of the Calderdale and Kirklees

Palliative Care Education Group

Improving working between organisational boundaries

Strengthening of a whole-system approach to end of life care

Lead and represent on discreet projects as directed
