Job Title: Primary Care HealthCare Assistant (HCA) for Long Term Conditions
- Responsible to: Service GP Clinical Lead
- Accountable to: Service Manager Haringey GP Federation
- Hours of work: Full time (Desirable), Less than full time hours can be discussed.
- Remuneration: £25K -28K (depending on experience)
- Contract length: Permanent
- Location: Working within General Practice in East Haringey including Community Centres.

Main duties of the job
We are looking to recruit a health care assistant to join the team and service. The role offers a rare opportunity to develop skills in an independent and supportive environment in a community health care setting within a new innovative team in Haringey.
The aim of this post is to support in delivering care to patients living with Long Term Conditions LTCs (Cardiovascular (CVD) and lung diseases (COPD) chronic kidney disease (CKD)) in the East of Haringey. Patients from BME backgrounds are much more likely to live in the East of Haringey and are at greater risk of developing CVD and CKD.
The post holder will work closely with the clinical leads to provide CVD/CKD screening by completing a health check to patients across Haringey and will focus on:
- Patients at high risk of CKD of CVD
- Completing a health check including phlebotomy

We provide a range of services in Haringey, including those which are best delivered by Primary Care at scale, namely; integrated services, working with partners in our local borough partnership, together with support services, including back-office support, workforce and training and some shared staffing clinical teams. We also currently manage two local GP practices and lead the North Central London Training hub.
