An exciting opportunity has arisen for a Primary Care Delivery Manager to join the Hammersmith & Fulham Primary Care Team in NW London to support 28 GP Practices across the borough.

The post holder will manage a portfolio of initiatives to support Practices and the wider Borough with priorities such as finance, contracts and workforce.

The post holder will work with key stakeholders including; ICB colleagues, Clinical Leads, local GP Practices, Primary Care Networks (PCN), Secondary Care Clinicians and Colleagues, the Voluntary Sector and third party providers.

The successful candidate will build and manage relationships with Practices across Hammersmith & Fulham and be a point of contact to support Practices with service and general queries. They will promote innovation and champion the development of new ways of working and service delivery by supporting the Associate Director of Primary Care, Programme Delivery Manager and the wider Primary Care team.

Main duties of the job

The post holder will be required to work flexibly to meet the needs of the team and will also be required to travel regularly across Hammersmith & Fulham for Practice visits. Previous experience working within the NHS, Health & Social Care Partnerships, General Practice or Primary Care is desirable as is previous project management experience.

Act as key contact for queries from practices and networks.

To support GP leads to focus on reducing variation in primary care, through the datasets available to primary care and foster a culture of continuous improvement.

Support the development of a standard approach to quality improvement for use with member practices to improve patients safety, patient experience and the effectiveness of commissioned services

Lead and promote excellent communication across the ICB, PCNs and its member practices to promote effective involvement and engagement.
