Job summary

The post holder will ensure accurate and timely reporting onto the regional (Care Room Register) and national NHS Digital system (Assuring Transformation). They will lead on data quality queries with providers and other stakeholders and liaising with NHSE/I teams to input and ensure uploads. They will work alongside the team’s data analysts to develop reporting for the lead contractor, NLFC governance system and NHSE/I.

A critical target the postholder will be responsible for is ensuring the clinical team are completing their Care and Treatment Reviews (C(e)TR) within the target timeframe. The postholder will be responsible for ensuring all aspects of the C(E)TR are in place and this occurs within the target. This will involve liaising with NHSE/I, providers and the clinical team to bring the panel together.

Your duties will encompass regular data input, data cleansing, maintenance of records and databases, taking and transcribing minutes at meetings, telephone liaison as necessary as well as providing annual leave and sickness cover for the Hub’s Administration Manager.

The post holder will be an active member and contribute to the effective operation of a multi-disciplinary team. Your dealings with matters relating to clients and staff is of a highly confidential nature and should not be divulged to another or discussed with other colleagues other than as necessary within the context of your role.

Main duties of the job

  • Working with the National Case Management System (NCMS) to ensure all patient information is up to date and accurate, running monthly reports ready for the business information team to reconcile this information with NCDR Reports.
  • Responsible for ensuring that the internal and external databases are kept up to date, ensuring all Transforming Care Patients and details are accurate. Meeting the monthly deadlines for the AT Register Submissions and bi-weekly deadlines for the Care Room Register system
  • Be responsible for the data entry and maintenance of the Transforming Care Dataset
  • Ensuring New Admissions and Discharges are updated on internal and external databases
  • Liaising with Case Managers and Providers to Co-ordinate and ensure C(E)TRs are booked in a timely manner
  • Escalating to Administration Manager when C(E)TRs may be delayed due to provider/Case Manager availability.
  • Monitoring and recording the outcomes of C(E)TRs
  • Be the main point of contact for all CTR related queries and admin lead for all LDA issues
  • Supporting data entry and management across the consortium (including AS)
  • Responsible for uploading Transforming Care patient documents onto the internal share drive and the National Case Management System
  • Attending and taking minutes for Transforming Care Meetings (weekly and monthly)
  • Organising Learning Events for Clinical Network Meetings, including finding a venue, organising lunches and refreshments, liaising with procurement to ensure venues are paid.
